2. Green Card/Green Slip
The NYSC Green Card, sometimes referred to as the Green Slip, contains important details from your NYSC registration. Print your Green Card and take it along with your other registration documents. It is advisable to keep both the original printout and photocopies.
3. School Identity Card
Take your valid school ID card to camp. The name and other details on your ID should correspond with the information used during your NYSC registration. Keep the original and photocopies available for verification where required.
4. Statement of Result or Certificate
You should also take your Statement of Result or Degree/HND Certificate, depending on the credential available to you. NYSC’s requirements vary for locally and foreign-trained graduates, so PCMs should make sure the academic document they present is consistent with their registration details.
5. Medical Fitness Certificate
A Medical Fitness Certificate is another important document to prepare before heading to camp. Make sure your certificate is properly issued and contains the required medical information. If you have a medical condition that requires special consideration, NYSC advises eligible PCMs to provide the appropriate medical report during registration.
6. Passport Photographs
Go to camp with about 10–12 recent passport photographs. It is advisable to have photographs available in the format and background required for your registration. Keeping extra copies can be useful because you may need them for different camp documentation.
7. Professional Licence for Medical and Other Health Professionals
Medical and other eligible health professionals should take their professional registration certificate/licence where applicable. NYSC specifically requires certain health professionals, including doctors, pharmacists, nurses, optometrists and other relevant professionals, to present their professional registration and practising licence.
8. NIN Slip, IT Letter, Medical Redeployment Report and Other Supporting Documents
PCMs should also prepare their NIN slip and any additional document that applies to their situation. Polytechnic graduates should take their IT letter where required. Those seeking medical redeployment should have the relevant medical report, while PCMs with an Addendum or Nerd clearance slip should also take the documents with them. Because requirements can vary depending on a PCM’s circumstances, it is better to carry every relevant document rather than leave an important document behind.
NYSC Camp Document Checklist 2026
Before leaving for camp, arrange your documents in a secure folder and check them one after another. Your checklist should include: Call-Up Letter, Green Card/Green Slip, School ID Card, Statement of Result or Certificate, Medical Fitness Certificate, 10–12 passport photographs, professional licence for medical/health professionals, NIN Slip, IT Letter for applicable Polytechnic graduates, medical report for redeployment where applicable, Addendum and Nerd Clearance Slip where applicable.
